In Episode 57 of "In the Interim..." Dr. Scott Berry sits down with Professors Victoria Cornelius, Danny McAuley, and Anthony Gordon to discuss PANTHER, an international adaptive platform trial targeting acute respiratory distress syndrome (ARDS).
PANTHER addresses a fundamental challenge in critical care: ARDS isn't one disease. It's heterogeneous, with distinct inflammatory phenotypes that may respond differently to different treatments. Traditional trials miss this, testing therapies on everyone and finding nothing works.
At admission, patients are randomized to hyperinflammatory or hypoinflammatory subphenotypes using a biomarker device, allowing targeted assessment for each phenotype. The primary endpoint is organ support-free days at 28 days, analyzed with a Bayesian proportional odds model.
Backed by NIHR, US DoD, CIHR, NHMRC/MRFF, HRB, and more, PANTHER’s shared infrastructure enables efficient integration of new therapies and ongoing collaboration. The episode explores the stratified design, modular protocol, operational hurdles, and de-risking industry involvement.
